How to use the Public Vaccination Portal

 

To be able to collect electronic certificates on vaccinations, taken tests or past diseases recognized throughout Europe, Ministry of Health instructed the Institute of Health Information and Statistics of the Czech Republic to create a public vaccination portal where every citizen can verify records of their vaccinations and taken tests:

How to log in at the portal?

1. Login with a one-time SMS passcode

 

a) Login with a one-time SMS passcode.

This login is a preferred option for people of Czech nationality who are included into Population Registry , have been vaccinated and provided their mobile phone number.

Enter your personal identification number and ID number into the relevant fields.

 

Confirm the information by clicking on the button Send.

 

You will receive an SMS with a code which you will enter into the One-Time Code field and confirm by clicking the Log in button.

If you do not receive the one-time code, please, choose another option to log in.

 

b) Login using a mobile phone number, email and date of birth.

This login is a preferred option for foreigners and children without an ID card, who have been vaccinated and provided their mobile phone number.

Enter your mobile phone number, email and date of birth into the relevant fields.

Confirm the information by clicking on the button Send.

 

You will receive an SMS with a code which you will enter into the One-time Code field and confirm by clicking the Log in button.

If you do not receive the one-time code, please, choose another option to log in.

 

2. Login via Identita občana

The guide on the Public Vaccination Portal

After logging in, proceed in the following way.

 

1. Personal card

After logging in you will see your personal card where, at the top, you will find your contact details (the information is received from the ISIN vaccination module) and can be changed by pressing the button Edit Contact Details.

At the bottom, you will find sections devoted to vaccination and taken tests. You can download the relevant vaccination certificate and the certificate of a taken test.

If your personal card contains incorrect certificates, information is missing or it is inaccurate, verify over the phone first whether the relevant medical facility (vaccination center, test site or general practitioner) entered your personal data correctly. Incorrectly entered personal data is the most common reason why some documents are missing.

 

 

2. Editing contact details

Click on the blue button Edit Contact Details on the Personal Card page.

 

New screen will open, where you can enter your current contact details.

Confirm the change by clicking Update.

*By changing the contact details in this public vaccination portal, you will also change them in the ISIN vaccination module.

 

3. Downloading a Vaccination Certificate

Clicking on the blue button Vaccination Certificate on the Personal Card page.

 

Will initiate download of your Certificate of vaccination in PDF format.

 

4. Downloading a Certificate of a taken test

Clicking on the blue button Test Certificate on the Personal Card page.

 

Will initiate download of your Certification of a taken test in PDF format.

 

5. Downloading a Certificate of a past disease

Clicking on the blue button Past Disease Certificate on the Personal Card page.

 

Will initiate download of your Certification of a past disease in PDF format.

A QR code issued on new certificates will be used to verify validity of the certificate via a mobile application.

 

 

FAQ – Frequently asked questions on the Public Vaccination Portal

 

My one-time SMS passcode login does not work. What should I do?

If there is a problem with logging in, please, verify over the phone first whether the relevant medical facility (vaccination center, test site, or general practitioner) entered your personal data correctly. Incorrectly entered personal data is the most common reason why you are not able to log into the system.

If your personal data was entered correctly and you still have problems with login, you will find the answer here.

How do I obtain vaccination certificate?

During the transitional period till 7.6.2021, all newly issued vaccination certificates will be automatically sent to your email after the first and second doses. During the week from 7.6.2021, the automatic sending of certificates to emails will be suspended and the Public Vaccination Portal will be used for obtaining the certificates https://ocko.uzis.cz, optionally it will be available upon request from the health service provider of the vaccination or test. After 7.6.2021, informational email will send, including details where the said person can pick up their certificate.

 

My contact details have changed. Can I update them myself?

Yes, if you need to change the registered email or mobile phone number, you can do it at the Public Vaccination Portal (the options how to log in are described above). Simply click on the button “Edit contact details”, change the information and confirm by clicking “Update”.

 

In my personal card there is a vaccination or test certificate missing. How is it possible?

If a certificate is missing, verify over the phone first whether the relevant medical facility (vaccination center, test site, or general practitioner) entered your personal data correctly.  Incorrectly entered personal data is the most common reason why some documents are missing.

 

Entering patient’s contact information – email and phone number

The data is entered into ISIN by the health service provider who makes the record of the vaccination, test and issues the confirmation. Only this provider can verify the patient’s identity and thus enter into the system their valid personal and contact information. IHIS CR is not able to change contact details based on a telephone or email request.

 

When and how can I view the certificate and print it?

After 1.6.2021 a Czech citizen can pick up a newly issued certificate (newly, not only a certificate of vaccination, but also of a past disease or taken test) according to the above-mentioned manual. This option will be available for foreigners in the week from 7.6.2021. It will be possible to view the certificate already after the first dose.

 

How will the exchange of old vaccination certificates to new ones occur after 1.6.2021?

After 1.6.2021, Czech citizens will be able to obtain new certificates at the Public Vaccination Portal https://ocko.uzis.cz (original certificates will no longer be available here, only new certificates). This option will be available to foreigners in the week from 7.6.2021.

After scanning the code of an old certificate, a unique URL will appear, it will be possible to download a new certificate under this URL (i.e. it is a 1:1 exchange) for any person who holds the certificate. It is a temporary solution without the need for further authentication (verification).

 

How will verification of a certificate’s validity occur?

Certificates validity is being verified by the verifier application. The application will be available approximately from the middle of June 2021. After reading a QR code from a certificate the application connects to the back-end IHIS CR and verifies the currently set validation regulations and, based on their assessment, the application shows information on the certificate’s status (valid/invalid). It is a temporary solution for validity verification of a certificate.

 

What are other future plans regarding options of obtaining certificates?

An option when certificates could be obtained at a Czechpoint is being prepared (indicative date for implementation is summer 2021).
